Professional Referrals

We make the referral process as straightforward as possible. Whether you're a GP, school, kinder, or allied health professional, we're a team you can trust to look after the families you send our way.
Online referrals
Simply access our referral form, complete it at your own pace. If you have any questions please contact hello@mayfieldhealth.com.au. Our team will take it from there and be in touch with the family shortly after.
What happens next
After we receive your referral, we'll reach out to the family directly to arrange an intake call. We'll keep you informed throughout the process with clear, timely updates so you always know how things are progressing.

Questions we're often asked.

Do I need to bring my child to a clinic?

No. If coming to a clinic doesn’t suit, we also provide outreach sessions within a 30-minute radius of our clinics at home, school, kindergartens, or childcares.

Who is Mayfield for?

At Mayfield Health, we support babies, children, adolescents, and their families across Melbourne's South East through our Frankston and Dandenong clinics.

We welcome children and young people of all abilities and provide support for a wide range of developmental, emotional, behavioural, and physical needs. Our experienced clinicians work alongside families to deliver goal-oriented therapy that is engaging, practical, and tailored to each child's unique strengths, needs, and aspirations.

We believe that meaningful progress happens when therapy is collaborative, purposeful, and centred around what matters most to the child and their family. Whether we are building communication skills, supporting emotional regulation, developing independence, or improving participation in everyday activities, our focus is on helping children and young people gain confidence, achieve their goals, and thrive in their daily lives.

I don't have NDIS, what do I do?

Navigating NDIS funding and payment options can feel overwhelming at times. Our team is here to support you through the process and help make things as simple and straightforward as possible. If you have any questions, please don't hesitate to reach out.
